Job Summary:
As a Licensed Nursing Assistant, you will be responsible for providing compassionate and personalized care to clients needing assistance with daily living activities; ensuring their comfort, safety, and well-being. Working under the supervisor of the RN assigned to the case and Clinical Director, you will support clients with tasks such as dressing, bathing, grooming, personal care, meal preparation, medication reminders, light housekeeping, and companionship.

Responsibilities:

  • Assist the client with personal hygiene (bathing, shampoo, shaving and oral hygiene)
  • Plans and prepares nutritious meals or modified diets as prescribed by a physician.
  • Assists with activities of daily living which include dressing, ambulation, toileting, and safe transfer techniques as directed by the Plan of Care.
  • Prompts client to take self-administered medications as per the care plan.
  • Works with other community agencies involved with the client as directed by the Plan of Care.
  • Assists with treatments/therapies as ordered by physician and approved and supervised by RN and Plan of Care.
  • Carries out assignments as instructed by HHA care plan and notifies RN when unable to do so. If RN is unavailable, notify the Director of Client Services.
  • Reports changes in clients’ condition or home situation immediately to the office.
  • Use of Mobile App for electronic record of visit, including activity, time, and mileage
  • Performs routine housekeeping duties as related to a safe and comfortable environment for the client as directed by the Plan of Care.
  • Accompany the client to necessary appointments, only when authorized by the office.
  • Respect the cultural and religious practices of the client and their families
  • Conduct oneself in a manner that promotes safety

Qualifications:

  • Previous experience in patient care, particularly in the home care setting
  • Have a valid New Hampshire LNA license
  • Compassionate and empathetic attitude towards clients
  • Strong communication skills and the ability to work well within a team environment
