Job Summary:
The LPN Clinical Coordinator plays a pivotal role in supporting clinical leadership team at Guardian Home Care of NH. This position is responsible for overseeing a specialized caseload, managing intake processes, supporting caregiver staff, and ensuring documentation, compliance, and communication are upheld across clinical functions. As a key point of contact for PCSP and HHA oversight, the LPN Clinical Coordinator helps maintain service excellence and operational flow from intake to ongoing care.

Educational and Professional:
  • Certificate as Licensed Practical Nurse from an accredited school
  • Valid License as a Licensed Practical Nursing in the State of New Hampshire
  • Five (5) years of Medical/Surgical experience as a LPN
  • Three (3) years of Management experience as a LPN preferred.
  • Home Health experience required.
  • Reliable transportation and the ability to travel from place to place.

Knowledge and Abilities:
  • Able to physically perform the duties required by the position.
  • Current CPR/First Aid certification
  • Knowledge of HHAeXchange EMR a plus but not required.
  • Experience in supervising and developing staff
  • Current driver's license and proof of automobile insurance
  • Communication and interpersonal skills to develop relationships with clients, staff, and providers
  • Skills: Nursing skills are defined by accepted nursing standards, preferred. Requires interpersonal skills and ability to manage patient care episodes.
  • Demonstrates strong verbal, written and interpersonal communication skills.
  • Strong computer skills.
  • Demonstrates time management, organizational skills, and ability to function with minimal direction.

Responsibilities:
              Clinical Oversight & Care Management
  • Oversee Licensed Practical Nurses (LPNs), and Caregiver teams (HHAs/PCSPs) in collaboration with Clinical Director
  • Determine the scope and frequency of services based on patient acuity and caregiver/family needs
  • Provide admission and direct patient care as staffing requires
  • Maintain availability of services by ensuring proper staffing coverage for all levels of care
  • Supervise and evaluate field employees, including on-the-job training and performance monitoring
  • Initiate, develop, and supervise individualized care plans and coordinate care between physicians, referral sources, families, and the care team
              Caseload & Clinical Coordination
  • Manage the caseload of clients who do not have a primary nursing authorization (Approximately 50 clients)
  • Coordinate and ensure completion of Q3 month supervisory visits and Q6 month recertification; delegate field visits to local RNs/LPNs when applicable.
              Intake & Documentation
  • Review all new intake paperwork for completeness and accuracy
  • Input clinical data into HHAeXchange (Diagnosis, medications, allergies, etc.) prior to nursing admission visits
  • Ensure documentation is accurate and supports compliance with agency and state regulatory standards
              Field Supervision & Compliance Tracking
  • Track completion of field supervision for caregivers and clinical staff
  • Monitor client hospitalizations and changes in condition; coordinate communication with hospitals, families, and case managers to update care needs
  • Act as a point of contact for clinical-related complaints and compliance concerns related to PCSPs and HHAs; assist in investigation and resolution in collaboration with Clinical Director
              Staff Support, Training & Skill Development
  • Oversee daily responsibilities and field practices of PCSPs and HHAs
  • Conduct staff competency assessments in the field once or twice monthly or as needed
  • Provide field retraining on hands-on care skills in response to performance issues or client-specific needs
  • Complete in-home caregiver skills assessment as part of new client onboarding/orientation in collaboration with Clinical Director
              Collaborative Responsibilities:
  • Assist Clinical Director in preparing for audits and surveys
  • Participate in QAPI efforts, staff meetings, and performance initiatives
  • Support emergency coverage planning and promote a collaborative, responsive care environment

Physical Demands:
  • Lifting, carrying, pushing, or pulling up to 50 pounds
  • Patient transfers with no assistance for direct patient care
  • Standing, walking, stair climbing, stooping, kneeling, and crouching while administering patient care in the home
  • Reaching and handling
  • Adequate eyesight for reading, writing, driving, and performing tasks

Aptitudes:
Have the ability to understand verbal and written instructions.  Read and write documentation in patient record. Work with hands and fingers to manipulate large and small objects.
